TRYOUT REGISTRATION

Tryouts will take place on Saturday August 10 at Ozaukee Middle School

12U is 9am – 1015am

13U is 1030am – 1145am

*Bring water, a basketball, and wear a reversible jersey for scrimmage.*

With the submittal of this form and waiver the below named parties hereby waives, releases and holds harmless Rise Skills Academy – its directors, organizers, coaches, volunteers or designees from any and all claims, damages, costs, actions and causes of actions as the result of personal injuries sustained as a participant in the tryout.

$20 registration fee required to tryout which is paid below…

Athlete will be notified within 48 hours of the result of the tryout.  Offers to join the team must be accepted within 24 hours.
